Creating a email-to form in dreamweaver 8

Hey,

I am working on a website for a hospital, and have hit a problem. I am trying to create a patient satisfaction form, where i have several radio buttons (5 for each section [very poor, poor, mediocre, good, excellent]). At the end of the form is a submit button, which when pressed, should go to a done page saying that it is submitted and will redirect you to the home page, AND send the form to 3 different e-mail addresses (3 admins at the hospital). How do I do this? I have the form part down, but I need some code to put in which will send the form, go to the done page, and then redirect to the home page. I am in desperate need of assistance, so any info at all would help a lot. By the way, all my pages are .asp files (if that matters).
