Question Simple FPS in Visual C++ 2008?

Hallo all... Well, I posted a pretty complete post.. but idk why it got erased when I clicked Preview ¨¨

So, I'ma make it simple, REAL simple:

1) How to create a camera behind my weapon model to make it look like FPS?
2) How to insert my own-made weapon models?
3) How to insert my own-made maps?
4) How to make that when LMB is pressed the animation of shooting plays? (also made by me)

All these models and animations are made in 3Ds Max 2011..

Thanks to all!
